RN-Caregiver Support Program

The Caregiver Support Program (CSP) mission is to provide comprehensive, inter-disciplinary support in the homes of Veterans (and their caregivers) with complex medical, social and behavioral conditions. The primary focus of CSP is longitudinal care for Vetrans with complex chronic disabling disease and effective support of their caregivers in the role of partner with the Veteran, to maintain a safe and therapeutic environment for the Veteran to remain at home. CSP team members work interdependently in assessing, planning, problem-solving, decision-making and implementing team tasks. These services are provided to male and female Veterans and Caregivers ranging in age from early 20's to late 90's.The RN functions in support of caregivers, Veterans and family members throughout the Medical Center and in community settings. and is responsible for:
l) assessing and reassessing the needs of veterans and care givers through routine in home and office-based assessments and 2) planning and initiating appropriate care 3) demonstrating clinical evidenced-based services and interventions for the caregiver and Veteran; to include education and consultation for the caregiver, Veteran, and staff on caregiver issues. 4) completing home visits, identifying problems and areas of concern, completing careful documentation of all findings and providing communication to the Caregiver Support Coordinator (CSC) to address issues that may impact Caregiver Program Eligibility. 5) completing the initial, quarterly, and annual in-home assessments for patients enrolled in the Program of Comprehensive Assistance for Family Caregivers (PCAFC) 6) facilitating educational caregiver support groups, responding to the Caregiver Support Line (CSL) referrals and participating in the planning/coordination of activities related to the annual National Caregivers month. The CSP RN will be assigned additional duties, as indicated by the CSC and/or his/her supervisor, which support the mission and goals of the CSP. This position is not suitable for telework. Not required Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Walking, bending, stooping, and some lifting in the direct care of patients. There may also be extended periods of sitting.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
